CWB Initial Appointment / Membership

Service Description

There is a lot to do in preparation for your new family member. After you book your initial appointment, you will receive a link to our client portal with full access to membership education. Now you can submit your medical history information to see if you will qualify to deliver with CWB. Please fill out this information as soon as possible to help staff prepare for your visit. It does take a bit to complete the forms and we recommend using a desktop instead of your phone. Please fill out all of the forms before submitting them. As part of your membership, you will have access to this portal throughout pregnancy and will also receive emailed information throughout pregnancy. You will also be able to see lab results. If you have started care with another provider we can request those records as well. We may also schedule childbirth classes or ultrasounds and we will work together to decide upon your plan of care. A non-refundable fee of $250 is required for this service but it goes toward your full fee. If you have private insurance, please also submit your insurance information including a copy of the front and back of your insurance card submitted to info@carolinawaterbirth.com. We gladly accept Medicaid Absolute Total Care plan. REFUND POLICY: We value your time and have a tight scheduling calendar so that we can be available to all of our families. Initial appointment fees are NON-REFUNDABLE. Please only schedule when you are sure of your schedule and decisions. Meet-the-Midwife days appointments are available FREE of charge to answer questions prior to Initial Appointments. You may wish to fill out a contact form on the website to verify that your due date is available. IF YOU NEED LABWORK, PLEASE SCHEDULE BEFORE 1PM. IMPORTANT NOTE: Other than for the initial visit, Carolina 'Birth no longer accepts credit cards. DIRECTIONS: We have recently relocated and Google Maps occasionally still directs people to the old location. Verify that your GPS device is taking you to 111 Commons Way in Greenville (NOT SIMPSONVILLE). ALSO, the turn on to Commons Way can be tricky. We suggest turning into Zaxby's and then turning left on to Commons Way.
